Plan Terms & Conditions
The 365 In-Office Savings Plan requires annual payment of $365 with immediate eligibility upon enrollment. The plan automatically renews each year unless you notify our office of cancellation before renewal. A $50 reinstatement fee applies if your plan lapses. You will not receive a membership card as your plan's effective date will be kept on file with our office. Annual rates may increase upon plan renewal without notice. Periodontal (gum disease) treatment is not included in the "Two Cleanings" benefit, and cosmetic procedures are not covered under this plan. The plan does not include advanced periodontal treatment, cosmetic procedures such as veneers or whitening, orthodontic treatment, oral surgery beyond routine extractions, or specialty referrals outside our practice. All plan benefits are designed to provide greater access to quality preventive and restorative dental care at an affordable price while eliminating the common hassles associated with traditional dental insurance.
